Intern Centers are required to have the following documents for Approval as Intern Center

1) List of Pharmacy staff (Full time and Part time) with their Academic certificates, Certificates of registration, License to practice, CVs and Contracts

2) Institution/Hospital Details (Including total number of beds available,Number of Patients (IPD & OPD) per day, Number of Wards and its capacity,Number of Departments/Units or Sections available and Number of Pharmacy sections available

3) List of required Internship reference and training tools

NOTE:
i)  Documents which are not in English Language must be interpreted by a recognized authority and attached to the documents of the original language
ii) ALL PAYMENTS ARE DONE VIA "CONTROL NUMBERS"
